For patients with ME/CFS

Introductory Masterclass

• A 20 week structured course walking mild-moderate patients through the basics of ME/CFS management

• Appropriate for: patients who are well enough to participate in group visits, who are not in crisis, who have minimal comorbidities, and who can travel to Terre Haute for a physical exam.

• Pricing: $1,500 which can be paid in monthly installments of $300 per month

• Includes

  • Physical exam in the clinic

  • Weekly group appointments

  • Ordering of labs (price of labs not included in cost of membership)

  • Three virtual one-on-one appointments (30 minutes each)

  • Prescription medications

  • Referrals to PT/OT

  • Many other non-prescription treatment recommendations with Fullscript access

• Not included (incurs additional costs)

  • Extra virtual appointments ($200 per hour)

  • Prior authorizations ($50 per hour, capped at $150)

  • FMLA and disability paperwork ($200 per hour)

  • After-hours communication and management ($200 per hour)

Gold Tier Membership

• For patients who need frequent visits and after hours communication

• Pricing: $500 per month membership fee

• Includes

  • Unlimited virtual visits

  • Unlimited group appointments

  • 1 house call per year (scheduling may be delayed to allow for batching of house calls)

  • Medication management, refills

  • Referrals for CFS-related issues

  • Interprofessional communication

  • PAs, paperwork, letters, and other between-appointment services

  • After-hours communication and management

Silver Tier Membership

• An ongoing membership for stable patients

• Pricing: $50 per month membership fee + additional fees as described below

• Includes

  • Virtual or clinic appointments ($100 per hour)

    • Alternatively, privately insured patients may submit invoices for insurance reimbursement and pay only their copay

  • Group appointments

  • Medication management, refills

  • Referrals for CFS-related issues

  • Prior authorizations ($25 per hour, including time on hold)

    • Alternatively, patients may submit their own prior authorizations. Instructions will be provided

  • FMLA forms, disability paperwork and letters of recommendation

  • Record requests and other between-appointment services

Bronze Tier Membership

• For patients who no longer need medication refills or ongoing appointments

• Pricing: No monthly membership. Additional fees as described below.

• Includes

  • Group appointments

  • Record requests ($200 per hour)

  • Virtual appointments ($200 per hour)

    • Alternatively, privately insured patients may submit invoices for insurance reimbursement and pay only their copay

  • FMLA and disability paperwork ($200 per hour)

  • After-hours communication ($200 per hour)
