What is included?

The package covers a full disability evaluation tailored for ME/CFS including:

Do you take patients from out of state?

Evaluations require that you travel to my clinic in Terre Haute, Indiana for the evaluation. Additional virtual appointment time for documentation is helpful but not necessary. You must be located in Indiana, Illinois or Kentucky for any virtual appointments. Otherwise, residents from other states are accepted.

Who can benefit from this package?

Anyone with diagnosed or suspected ME/CFS seeking a thorough disability evaluation and clear documentation.

How much does this service cost?

The evaluation costs $800. Payment plans are offered to those who cannot afford to pay that up front.

$150 is due at the time of enrollment and is non-refundable. The remaining $650 is paid at the time of the first appointment or a payment plan is negotiated.

How long does it take?

Evaluations typically take a few weeks from start to finish. Appointment are usually booked out only 2-3 weeks so assessments can be done without delay.

How do I get started?

I recommend downloading the Disability Evaluation Package files below and reviewing the service Agreement and Patient Form. You can take your time getting the Patient Form filled out. When you are ready, sign up using the enrollment link.
